Jaclyn Fermo, NP, is a board-certified Family Nurse Practitioner specializing in primary care and cardiology. She earned her Bachelor of Science in Nursing from SUNY Plattsburgh and her Family Nurse Practitioner degree from St. John Fisher University. Prior to becoming a nurse practitioner, Jaclyn worked as an ICU nurse, caring for critically ill patients during the COVID-19 pandemic. Fluent in both English and Tagalog, she is proud to connect with patients from diverse backgrounds as a first-generation immigrant.

Jaclyn’s passion for preventive medicine grew from her frontline experience caring for patients with complex, often preventable chronic conditions. She chose family practice because it allows her to care for patients across all stages of life, from children to older adults, while her interest in cardiology developed through years of critical care nursing. Her approach to treatment is centered on collaboration, education, and prevention, with a focus on helping patients understand their health and make informed decisions. Jaclyn believes patient care means treating the whole person—not just a diagnosis—and building trusting relationships that support long-term health and wellness.
